Online Assessment Procedures for the Doctor of Pharmacy Distance Pathway Exam Restriction Form Instructors in the Doctor of Pharmacy Distance Pathway employing online exams complete an Exam Information Form and an Exam Restriction Form for each exam to be given during the semester.
Proctor Sites Doctor of Pharmacy students in the distance pathway are required to use an approved exam proctor for many course exams. Instructors will inform students of exams that must be proctored. Students use testing centers that are members of the National College Testing Association (www.ncta-testing.org). The Distance Pathway Office identifies approved testing centers within a 30 mile radius of each student's home and notifies students of their exam proctoring location.
Exam Materials At least three weeks prior to the beginning of a testing window, all exam materials for a proctored exam must be turned in to the Distance-Pathway Office (Andrea Winters at 280-1282, Criss III room157 E) so that they can be uploaded or mailed to proctors.
