Procedures Related to Administration of Online Assessments


The following procedures address the administration of online assessments within the School of Pharmacy and Health Professions.

Emergency Procedures
Procedure to follow in the event of an emergency during an online exam are outline on the Emergency page located at http://spahp2.creighton.edu/distance-ed/Emergencies.aspx.

The procedures for creating online assessments differ depending on the tool used. Follow the links in the menu on the left to each assessment tool to view the assessment creation procedures for that tool.

Online Assessment Procedures for the Doctor of Pharmacy Distance Pathway
Exam Restriction Form

Instructors in the Doctor of Pharmacy Distance Pathway employing online exams complete an Exam Information Form and an Exam Restriction Form for each exam to be given during the semester. 

  • Forms are completed prior to the start of each semester. 
  • Department administrative assistants will contact faculty when it is time to complete the forms. 
  • The form identifies the parameters for each exam such as whether scratch paper is allowed, if calculators are allowed and it so what type, the time frame each exam is available to distance students, and procedures to be followed in the event a distance student is unable to complete an online due to unforeseen technical problems.  
  • To ensure there is sufficient time to communicate exam restriction changes to all proctor sites prior to announcing changes to the students, changes to exam restrictions must be communicated to the Distance-Pathway Office no later than 10 working days prior to the beginning of a testing window.  

Proctor Sites
Doctor of Pharmacy students in the distance pathway are required to use an approved exam proctor for many course exams.  Instructors will inform students of exams that must be proctored.  Students use testing centers that are members of the National College Testing Association (www.ncta-testing.org).  The Distance Pathway Office identifies approved testing centers within a 30 mile radius of each student's home and notifies students of their exam proctoring location. 

Exam Materials
At least three weeks prior to the beginning of a testing window
, all exam materials for a proctored exam must be turned in to the  Distance-Pathway Office (Andrea Winters at 280-1282, Criss III room157 E) so that they can be uploaded or mailed to proctors.
