An unofficial prerequisite evaluation helps students know which of our prerequisites have been fulfilled with their graded, in progress, or planned courses. We are not able to identify courses for you to complete to fulfill deficiencies; however, we are able to evaluate courses you provide for review. At times, it may be necessary for us to request a course syllabus.

It is strongly recommended that you have your science prerequisite coursework almost complete before requesting an unofficial evaluation.
Prerequisite courses must be completed with a grade of C or better (a C- grade is not acceptable). Coursework from institutions that evaluate students with a narrative rather than numeric or alpha grades is not accepted.
Submit your official or unofficial transcripts by e-mail to phaadmis@creighton.edu or mail. We do not accept faxed copies of transcripts due to legibility and transmittal issues.
Please include (either in separate document or in the body of your email) which of your courses you think will fulfill our prerequisite requirements and a course description for each of these courses. Course descriptions can usually be found through your institution's electronic course catalog. Please be sure to provide descriptions from the year when you were enrolled in each specific course.
Transcripts must identify the name of the institution.
You must include your legal name, mailing address, telephone, e-mail, and intended program of study.
Incomplete requests will not be processed.
Due to the heavy volume of evaluation requests, please allow up three weeks for the results of your evaluation to be sent via e-mail. Transcripts will not be returned. Please note that this is a service provided by the Office of Admission and requests that have unique circumstances involving the need to inquire with the admission committee may take longer.
Fall 2010 Applicants: If requesting an unofficial evaluation within the same academic year of intended application, the Office of Admission reserves the right to deny the request on the grounds that an official evaluation will be completed at the time of admission review.
